Pathfinder – Adult Social Care Early Intervention Prevention Service

The newly commissioned Pathfinder service provides a single point of access to over 26 different early intervention and prevention services. These services range from low level community support to reduce social isolation through to more specialist services to support people to remain as independent and as well as possible for as long as possible.

These services include:

Community well-being
-Group social activities
– Befriending schemes
– Community development
– Assisted shopping
– Support for small charities and social enterprises

Specialist support and activities
– Mental health services
– Supported employment
– Learning disability and autism services
– Sensory impairment support
– Falls prevention

Reablement
– Supported hospital discharge
– Home support for those with life-limiting conditions
