Primary Care DVT Service

A locally developed Primary Care DVT Service enabling patients with suspected DVT to be seen by their GP and referred to a primary care based local service, outside of hospital. The patient attends their GP practice with a painful and/or swollen leg. If the GP suspects a DVT then depending on the level of service the practice is offering the patient receives a primary care based service but may have to travel to another practice for some elements.
NB – there are some clinical exclusions to the primary care service.

All patients previously would have been sent to the Mid Cheshire Hospitals Foundation Trust (MCHFT).

The service operates for the GP core opening times and outside of these hours the patient is likely to present or be directed via 111 to MCHFT.

0 The GP suspects a DVT and refers the patient to the DVT hub who provide level 1 and any further care.
1 The GP suspects a DVT and completes a Wells clinical assessment and a point of care Di-Dimer blood test. If these indicate a risk of DVT the practice refers the patient to the DVT Ultrasound Service (DVTUSS) which is co-located with the DVT Hub. If the patient has a DVT then they are seen by the DVT Hub for treatment and referred back to their own practice for any follow up investigations.
2 The GP suspects a DVT and completes a Wells clinical assessment and a point of care Di-Dimer blood test. If these indicate a risk of DVT the practice refers the patient to the DVT Ultrasound Service (DVTUSS). If the patient has a DVT the DVT USS informs the practice and the patient’s own practice initiates treatment for the DVT and arranges any follow up investigations.

Project Funding

The CCGs commission this service from the South Cheshire and Vale Royal GP Alliance who sub contracts with the GP practices across South Cheshire and Vale Royal and the DVT Hub. The DVTUSS is directly commissioned by the CCGs and works in partnership with the Practices and the Hub to deliver seamless patient care.

Project Implementation Date

The service commenced in June 2016 and is a 2-year contract.
